        Identity and expression are significant themes in sex therapy covering comprehensive sexuality, personal sexual identity, and gender identity and care, intertwined with humanity as a whole. Recognizing the influence that it has on relationships, mental health, and sexual wellness, sex therapy helps build a culturally inclusive and diverse sex therapy experience (Berry & Lezos, 2017). A safe space that is affirming helps clients explore their identity and how to express themselves without feeling judged or fearful of asking questions or talking about sexual identity and expressions.

         A major focus in this theme is providing sex therapy for LGBTQAIQS+ and queer-affirming care. Those who identify as lesbian, gay, bisexual, transgender, queer, questioning, asexual, intersex, and other gender identities (LGBTQAIQS+) face unique challenges related to discrimination, love and acceptance, family, being a minority, discrimination, and accessing affirming healthcare (Reczek, 2020). Sex therapists help clients navigate concerns that arise, including those related to mental health, sexual functioning, belonging, diverse relationships, and healthy ways of coping, leading to self-acceptance (Heredia & Rider, 2020).

 Providing gender care is a key area in sexual health. Exploring gender identity and processing life experiences and transitions, and what that looks like socially and physically. Sex therapists provide support by working collaboratively with clients and practitioners. Understanding gender dysphoria and body image is significant when helping clients and partners work through their experiences (Heiden-Rootes et al., 2023). Likewise, therapists support intersex and those with identities outside of the traditional binary knowledge of gender (Association, 2015). Those who are born with various sex characteristics may encounter even more complex challenges (Urone et al., 2026). Non-binary clients need support in ways that help them express their gender outside of exclusive male and female roles. Providing an affirming, non-judgemental space supports clients while cultivating authenticity and resilience.

         Fundamentally, the goal of affirming identity and gender sex therapy promotes empowerment and sexual wellbeing. It is important to honor diverse orientations and gender expressions as therapists helping support clients leading fulfilling lives. Inclusive, affirming care in sex therapy helps individuals live to be their true authentic self (Berry & Lezos, 2017).
